What is a Community Health Centre (CHC)?

Community Health Centres across Ontario provide Health Care services that are delivered by a team of professionals consisting of a mix of Physicians, Nurse Practitioners, Registered Nurses, Health Promoters, Community Outreach Workers and other support professionals. As a patient of a Health Centre you will receive care and support from the whole Team. 

When you make an appointment, you will be booked with the medical professional who has the right skills to meet your needs.

Who funds the NFCHC?

We are funded by the Ministry of Health and Long-Term Care (MOHLTC)

Does NFCHC have a walk-in clinic?

NFCHC does not provide emergency care, nor is it a walk-in clinic. Patients must make appointments during the week, however, same day visits may be available if needed. Also, NFCHC offers an Acute Care Walk-in Clinic on Saturdays for already registered patients to come and have their issue seen on a walk-in basis. Please contact the CHC to check availablilty.

Can I use your services if I don't need a Doctor or Nurse Practitioner?

Absolutely! With the exception of our lab, our programs and services are open to residents of Niagara Falls, regardless of whether they visit a member of our medical team.

Do I have to pay to receive service?

No, there are no fees for our services.

What are the types of health professionals and other staff that provide service?

The NFCHC has Family Physicians, Nurse Practitioners, Registered Nurses, a Dietitian, a Social Worker, Psychotherapists, Outreach Workers and a Health Promoter. We also have a COPD team which consists of a Certified Respiratory Educator (CRE), Occupational Therapist, and Kinesiologist. 

What if I miss an appointment?

Patients are asked to make every effort to keep appointments and to be on time. If you cannot make an appointment, we request 24 hours notice.
